Calverton Footcare – Terms & Conditions
1. Agreement
1.1 These Terms & Conditions govern the provision of services by Calverton Footcare (“I”, “me”, “my”) to you (“you”, “your”).
1.2 By booking an appointment, you agree to these Terms & Conditions.
1.3 If there is any inconsistency between these Terms and other written information provided by Calverton Footcare, these Terms will prevail. Your personal information will be handled in accordance with the Privacy Notice.
1.4 I may update these Terms from time to time. Any changes will apply to future appointments and services.
2. Your Information
2.1 You must provide accurate and complete personal, contact and medical information relevant to your treatment.
2.2 You are responsible for notifying me promptly of any changes to your contact details, medical conditions, medications or allergies.
2.3 I am not responsible for missed communications where incorrect or outdated contact information has been provided.
3. Appointments
3.1 All appointments are subject to clinical assessment.
3.2 It is your responsibility to select the most appropriate appointment type. If an incorrect appointment is booked, treatment may need to be modified, rescheduled or declined.
3.3 I reserve the right to refuse, postpone or modify treatment where it is not clinically appropriate, safe, or within my scope of practice.
4. Fees and Payment
4.1 Fees are payable at the time of booking or on the day of treatment unless otherwise agreed.
4.2 Accepted payment methods are debit card, bank transfer and cash. Cheques are not accepted.
4.3 Outstanding balances must be settled before further appointments are booked.
5. Third-Party Payments
5.1 Where a third party has agreed to pay for your treatment, written confirmation must be provided before the appointment.
5.2 If the third party fails to pay within 30 days of invoicing, you remain responsible for the outstanding balance.
6. Cancellations and Non-Attendance
6.1 Please provide at least 48 hours’ notice if you need to cancel or reschedule an appointment.
6.2 A charge may be applied for late cancellations, missed appointments or repeated failure to attend.
6.3 If you arrive late, your appointment may need to be shortened or rescheduled.
6.4 Repeated non-attendance may result in future appointments being declined.
7. Assessment and Treatment
7.1 Treatment recommendations are based on my professional assessment of your condition and individual circumstances.
7.2 You are responsible for ensuring that you understand the proposed treatment and raising any questions before treatment proceeds.
7.3 Failure to disclose relevant medical information may affect the safety, suitability or effectiveness of treatment.
8. Clinical Outcomes
8.1 All treatments are provided with reasonable care and professional skill.
8.2 No healthcare treatment can guarantee a specific outcome, and recovery times may vary between individuals.
8.3 Any material risks associated with treatment will be explained before treatment is provided.
9. Children and Young People
9.1 Patients under the age of 18 must have consent from a parent or legal guardian.
9.2 A parent or guardian may be required to attend appointments and remains responsible for payment of fees.
10. Confidentiality and Data Protection
10.1 Personal and medical information will be handled confidentially and in accordance with applicable data protection laws.
10.2 Personal data is processed in accordance with the UK GDPR, the Data Protection Act 2018 and my Privacy Notice.
10.3 Health information is processed only where necessary for the provision of healthcare services, legal obligations or other lawful purposes.
10.4 Clinical Photographs
Where clinically appropriate, photographs may be taken as part of your healthcare record to assist with assessment, treatment planning, monitoring progress, and maintaining accurate clinical records.
Clinical photographs form part of your confidential medical record and will be stored securely in accordance with applicable data protection laws and my Privacy Notice.
Any use of photographs for purposes other than your direct care, including training, education, marketing, or publication, will only take place with your separate explicit consent.
11. Conduct
11.1 Patients are expected to behave respectfully towards staff and others at all times.
11.2 I reserve the right to refuse or discontinue services where there is abusive, threatening, inappropriate or unsafe behaviour, safeguarding concerns, or repeated breaches of these Terms.
12. Ending or Refusing Services
12.1 I may refuse, postpone or discontinue treatment where:
treatment would be clinically unsafe;
sufficient information has not been provided;
fees remain unpaid;
these Terms are repeatedly breached; or
a professional therapeutic relationship cannot be maintained.
13. Liability
13.1 Nothing in these Terms limits or excludes liability where it would be unlawful to do so, including liability for death or personal injury caused by negligence.
13.2 Subject to clause 13.1, Calverton Footcare’s liability is limited to losses arising directly from the negligent provision of services.
13.3 I am not responsible for outcomes resulting from a failure to follow clinical advice, inaccurate information provided by you, or decisions made outside my recommendations.
